Plaintiff, Pedro Ramirez’s attorney of record, Raymond Ghermezian, moves to be relieved as counsel. Counsel declares the attorney-client relationship has deteriorated such that further representation is not possible.

Counsel declares the moving papers were served on Client at his last-known address. Counsel fails, however, to declare either (a) he confirmed the address, or (b) the motion should be granted despite his failure to confirm the address. Either ¶3b(1) or 3b(2) of the declaration must be filled out. Because it is not, the motion is denied.

Counsel is ordered to give notice.
