Plaintiff Dean Wallraff (“Plaintiff”) requests that the Court enter default judgment against Defendant Abbas Homaizad. Plaintiff seeks an award of $33.273.13, reflecting $31,848.13 in damages and $1,425.00 in costs.

The Proposed Judgment submitted by Plaintiff is incomplete. The box under Section 1(e) has not been checked. Further, the box at the top of page 2 of the Proposed Judgment has been checked incorrectly (clerk judgment instead of court judgment).

The Court orders that Plaintiff to correct the above deficiencies and bring the corrected Judicial Council form judgment to the hearing.
